i did it! i prefer tv but the only scheduled tv i watch is the news and special shows like the apprentice and top gear. i rely heavily on tv on demand :)

would help. I'm beginning to like online streaming more & more since I bought my PlayBook. :D
There was a question on switching streaming services if it cost less. The problem is what if it cost less, but doesn't have everything I want ?
Also when I watch non streaming TV, it has been TiVOed, so scheduling and commercials are irrelevant.
Filled it out,
one question that was missing was
"do you calculate the cost of your increased internet connection requirements when selecting an online streaming vs TV on Demand service"
I don't do any online streaming, as if I am going online to get content, I am downloading it and watching it locally, that way I have control of the schedule and am not limited to internet connectivity, or devices that require internet connectivity.
I would have done your survey except you are stupidly asking humans what their gender is. Humans have a sex and words have a gender. Are you in university?

Friendly tip: for future surveys, you may want to "pretest" your questions or otherwise hone them before using them to collect data. For example, in this survey, all questions require an answer, yet for a number of them none of the choices applied in my case so I was forced to either abandon the questionnaire or provide an incorrect answer. Also, due to the wording there is some ambiguity across the questions with regard to whether "TV" means "television content broadcast and received by a television set" or "television content streamed via http and received by an internet-connected device". The more precise your questions, the more accurate the data collected ;-)

Not to mention that your survey didn't take into account usage of DVR's -- it isn't exactly streaming, neither is it on demand video
