The BOSE ones are the best I've found. Normally I HATE their products, but I did a lot of blind tests and they stood up way against the others!

Personally, I can't stand anything in my ears. I use the Motorola BT Stereo headset (HT820), over-the-ears. They're not compact but, very comfortable and it puts out a great sound.
