Typically these two positions won't be held by the same person, especially since part of the role of an executive chairman is to monitor and evaluate the decisions of the CEO and basically make sure they don't do anything stupid..

Is this a Canadian thing? Or something that became necessary to speed up decisions?

Time wasn't on their side. Fast, hard decisions with little time to consider and negotiate alternatives.
With water coming in fast, I would have to give the plumber, hired to fix it, the benefit of the doubt, and let him do his thing.
