Try a different set of headphones. I remember some tours had something wrong with the jack also. How long have you had the phone, is it new? used?

If you had to jam the headphone connector into the headphone jack, you may have bent one of the contacts. Take a look into the jack and see if anything looks bent or out of place.

That would be the first thing I'd look at anyway.
