You can either pay verizon for the service or you can buy an app to connect. Buying an app is cheaper but It constitutes a bit of a risk because it violates verizon's tos.

There are different plans for different amounts of money. I could tell you $500 but you won't know for sure unless you actually call verizon or visit a nifty kiosk
VZW cost is between $15-30 depending on which data plan you are on. Basically it all ends up costing you $59.99 a month for device AND tethering access ($30 + 30, or $45 + 15)
