I'll delete a ringtone, then go to sounds to set alerts just to find the ringtone file name again. How do you get rid of it completely?

if you don't delete the ringtone, does it add a second entry to it if you connect the phone to the PC again (this is an issue I'm having and trying to sort out, sounds like its along the same lines.)?

I haven't connected it to the pc, I just delete them from my phone and find them again.
