I just received a replacement STORM... they did take the SIM & MEDIA CARDS out of mine and put them into the new one. When I got home I just restored a back-up that I had made using DM.
The Device Switch in DM is more for if you are changing device types, so it does not need either of the cards. DM would basically do a backup of the old Phone (less any device specific information) and then do a restore to the new Phone - basically just data.
If you are going from one STORM to another, the backup and restore feature may work better. But make sure to backup your applications using BBSAK...
