Just found the $50 mail-in-rebate form on verizonwireless.com
Steps to locate it:

1. Go to verizonwireless.com and search for 'rebates'
2. The first result is the 'Verizon Wireless Rebate Center' - here's a direct link if you just want to go straight there
3. Scroll down to the section where you can enter a promotion code, type in '08-022', and then click 'Continue'
4. This will bring up a ton of results. Keep clicking 'Next' till you get to around the 6th page... there you will see 'RIM Blackberry Storm ' (I think it's the 3rd item down).

I'm not that familiar with this site, but it looks like it let's you type in your name and address and prints out the rebate form with it all filled out for you. Then you just have to mail it in.
