I've installed it twice and restarted using QP both times with no results.

That's very strange.

So in the Application Menu you're essentially getting the icon grid from the default theme? A battery pull almost always solves this problem.

Try:

1) Uninstall AppleBerry
2) Load up the default theme.
3) Reboot your device.
4) Reinstall AppleBerry.
5) Load up AppleBerry.
6) Restart your device.

If that doesn't work, let me know what OS you're running.
