My wife and I both have Q5 phones and both have 10.3.1.1565 installed.
Her phone. fully charged at night is almost fully drained by morning whilst my phone is fine.
How do I check what might be causing it?

Go into device monitor and have a look what's pulling so much power.

It's probably the BBM battery drain issue. Go to device monitor. In the battery tab, select "last 48 hrs" in the top right and post a screen shot here including the battery graph. It'll help identify what's draining the battery.

Please post a screenshot of your wife's device monitor screen. A comparison is not relevant unless every app/data are the same on both devices. Also, without the graph, it's impossible to know how many times the phone was charged and it's hard to speculate.

And it's probably the PIM services but can't know for sure without the battery graph.

Does she have a lot of accounts syncing at once in the hub? I'd suggest to remove them, re-add them one by one, and change the sync internal (24 hours or manual).

If that doesn't help the battery drain issue, as someone else mentioned, a security wipe may be the way to go.
