Hard to say.. but given it's a 5-year-old device, can't really count that out. Have you tried a factory reset?

Have you replaced the screen? There will be a rubber gasket that has to be transferred over from the old screen to the new one that focuses the proximity sensor to a small aperture at the front, and without this, it will be stuck in "near" mode all the time.
