when you enter development mode (like to install android player) you'll have to set a password, and if a password was set before and disabled, it will be enabled..
if you don't know that, you'll have to wipe the device :S
i think entering the wrong password twice more will do that for you anyway

hmm..i'm installing new os..huh..arrghhh..need to learn how to install android market for pB...
The screen does tell you to enter "blackberry" to continue... Check your screenshot.
Just type "blackberry" without the quotes and hit OK.
Reading can go a long way!
That's just RIM's way of making sure that the keyboard is working correctly and that the keys aren't randomly being entered. As soon as they enter "blackberry" they will again be prompted to enter a password.
If development mode was activated then you had to create a password. The only thing I can tell you is what we tell people at work who get locked out of their BB, I hope you backed up sometime recently.......
