I want to call an international number, so I open contacts, hit the number, the phone intuitively dials a local access number and routes the call avoiding expensive cellular rates.

Have you considered using a sip client like sip.fm with an ITSP?

The dialer app "BlaCaDa" (BLAckberry CAlling card DiAler) is as well integrated with the Contacts as the BB10 OS allows. It uses the "Share Contact" menu. You can call also from any location (e.g. web browser, e-mail text, etc.) where you can highlight a phone number and select the "Share" menu. The Screenshots in the App World provide some examples.
