I guess it would all depend on, what type of person are you? Safe or a risk taker? Are you a klutz or you never drop your phone.
Phones are a tad cheaper a few months after release, and I buy my phones outright to keep the cell phone plan cheaper and a month to month.
So you have to weigh out the price you're paying for the insurance: how much per month are you paying, then how much is the payout to get a new phone should you need a new one, vs how much is the phone that you can buy.
If your insurance is $10/month and a $150 payout to get a new phone... after two years the insurance will cost $390 and you can purchase a good new/used Passport for roughly $400. You'll save $!10, but what will the price of the Passport in two years and will you still want the Passport or something newer?

Get squaretrade. I doubt that asurion actually stocks the passport so who knows what you'll get. Squaretrade will special order you the phone if they don't have it in stock.
I had an original and dropped it in the pool. Went to ATT to see my option with a warranty. They charged me $150 and gave me a used ATT version...so FYI.
