No, no adapter needed. One cable. USB 2.0 + HDMI in one port.

You should already have a regular HDMI cable if you have an HD TV/screen.

With the current devices, you need either to buy a separate HDMI to micro-hdmi cable, or a micro-hdmi to HDMI adapter to use your existing HDMI to HDMI cable.

The only issue I see is for those that hook up both to an HDMI monitor and want to use USB keyboard/mouse. Is there a special adapter that allows for both simultaneously?

Try this instead. But just so you know, how would you have connected if you didn't have to buy an hdmi if you don't have a wireless system? Which means the only extra you "might have" had to get is the adapter. But as you will see in this link you actually won't even have to do that

The MHL adapter needs an external power dongle?
If true BlackBerry has severed it's own head. The BlackBerry Passport must support micro-HDMI because video via USB sucks.

No. Newer systems don't require it. Think of a slightly thicker USB 2.0, with a wider output "dongle"

No power adapter necessary, although one can use one to charge the phone at the same time.

This is the minimum Slimport setup:

Agreed. This blog summarizes well the issues with the various implementations of video out on modern devices that have foregone true HDMI support.

Even TV compatibility will become an issue without a dedicated HDMI port.
