Camera may be defective.
Reboot the phone, tap on the camera a little, it might help.
Otherwise replace the camera.

I had the same problem and replaced on warranty. You could replace on your own the camera because it's a hardware issue or replace the phone on warranty.

You guys are killing me.

The passport is KNOWN to have a ****ty focusing camera. It's a software defect. I've heard you can download camera++ and that will constantly auto adjust the focus, but my busted BlackBerry doesn't allow me to buy apps in BlackBerry world any longer due to a broken PayPal connector.
