I got a unlock code and every time i try it in leap it doesn't work. Could it be a wrong code or am i doing something wrong? how do i reset the attempts? should sim card be in phone to unlock? I need to be set strait before i run out of trys.

If an unlock code fails more than one time and you are certain the code is entered properly then a new code is needed. Or something is wrong with the process. A correct code entry should work the first time entered. Attempts cannot be reset.
