I has DNS66 working until the July update messed it up.

Never heard of that one. I have only used Adguard and DNS66 on the keyone. Both worked great until July.
I gave up on the keyone and went back to my pixel xl awaiting the notification fix and hopefully the fix to allow the VPN to run again in the background.
I use NetGuard and have the watchdog mode enabled, which will restart its VPN service if Android nukes the app in the background (which is the root cause of the "notification issue" ). Trick is, you buy NetGuard on the Play Store, then install the version from the github site. Both are signed w/ the same certificate, so the Play Store will recognize both. You have to use the version from github if you want the hosts file blocking capabilities, due to Google's policies regarding ad-blocking apps on the Play Store.
Using this, you can not only get hosts file-based blocking, but also set custom DNS servers (e.g., OpenDNS), and get a firewall that allows you see which domains each app visits and then allow or block that access. For example, I've set blocking rules in place for most apps that try to connect to Facebook's "graph" service, except my browser, since that is conceivably the only app I might ever need to visit FB in.
Set the background data authorisations for the apps to authorise.
Then check battery settings don't force them to sleep
