Little confused here. Exactly which profile option do you set for a custom tone notification when you get a new voicemail? Is it Level 1?

Your voice mail is SMS text they are the same....

A Level 1 Message is any message that meets the criteria you set to be Level 1. For example, I can create a Filter that sets any message with 'High' priority to be Level 1. I then go into my Profiles and set the Level 1 messages to have different notification than regular messages.
