If someone sends me a MMS message with Audio and I listen to it, it kills sound to my ear piece. If I make a call or receive a call I cannot hear anything. I have to do a battery pull to get it back to normal.
That is a known problem with some types of audio files. The problem occures if you interrupt it during play back, it fails to release the speaker. If you let it play to completion, it works fines.
The quickest way to get the speaker back is to use Voice Activated Dialing. That will release the speaker.
It has been reported, that problem has been corrected in the v4.5xx upgrade.
