The indicator light still works fine, but the lens is missing, has this happened to anyone else?

No mines still there. You might have dropped your phone and the thing came loose. By my knowledge that little clear cover is glued onto the front plate.

So a new case should have a new led cover?

Yes. You don't have to buy a whole new housing. Just the front plate. Unless if you want to change the whole housing to a different color.
