    There might be a hardware problem. Try bbsak, do a factory reset and wipe the phone. (Do both.) Make sure nothing remains on the phone. Then load the latest offiicial OS (I'd prefer 540).

    If it doesn't fix, send your device for replacement

    I had this issue before and ended up wiping all the data on the media. Well good news is that after a quick format (not reinstall OS), its all good
