Whenever I reset the phone normally or after a battery pull or after I install a new app, my blackberry menu icons shift so that I have to move them back to where I want them. Also, any folders I have created do not appear again after the restart. Rather annoying. Is there a reason or fix for this issue?

You are using a Today or today plus theme from an older OS most likely. Those older themes do not work with the newer 4.5 OS's.
