Are you using Desktop mode?
That icon is just a web shortcut.

That "app" is just a Web shortcut. It opens the Browser to the Youtube page. Just look at the address bar at the bottom, it's the Browser.

If you have Desktop mode set in Browser setting, disable it.
Remove cookies and history to make sure you do not have any old/bugged youtube cookies.
Make sure you do not have Youtube set to only use flash, if that is even possible, the browser no longer supports flash.
