I had a 3GS, jailbroken... downloaded fake carrier. Change the carrier to my name.
Updated to iOS4, phone was no longer jail broken, so I didnt have the app anymore, but my name was still on the phone as the carrier.
just updated to iphone 4, not jailbroken, synced phone and put the backup on it that was from my 3GS, STILL HAVE THE FAKE CARRIER NAME.

anyone know how to get rid of this?

I saw a similar thread like this on macrumors the other day. They found the answer to your question. Google

So why didn't my iPhone fully get rid of the jailbreak?

    I would post a link, but I don't have 10 posts yet. :-(

Do you still have the app, because all you have to do is go into the app, click fake carrier then hit reset and it should clear what ever you wrote.

I don't have the App, it's a jailbreak app and as far as I know iPhone 4 running 4.0 isn't jailbrokem yet, probably just a matter of time.
