Default questions about upgrading: Family Plan, replacement, and more

I've been with Tmobile using the 8320 since March 2008. from that date, I would have been eligible for upgrade by now. I have a few questions about this.

almost a year ago exactly my (now) husband joined our accounts into a Family Plan to cut costs. will this effect me being eligible for my 2-year upgrade?

I also had to get a replacement last summer. I have insurance on my device and was not charged anything for this. will this effect it too?

and lastly: how do you qualify to put the cost of your upgrade on your monthly bill?

Sign to your My T-Mobile. I was just looking at it this morning. It will show
you the upgrade price for the new phone you are considering buying, and the
amount you pay if you use the Installment Plan.

You have to pay one month payment plus your regular monthly bill amount,
and the rest will be billed in installments on your bill.

I know how to place the upgrade order, I just don't know if I'm still eligible. I don't have the authority to upgrade it on my account.

Smartphone line should be eligible for a full discount every year now since the upgrade policy change.. someone correct me if I'm wrong here.

Also you should be able to put the upgrade on your bill either all at once or in 4 payments. You should ask whoever is the primary to add you as an authorized user.
