The "Extra initialization commands:" field should have the exact string shown below.

Then go to Start->Connect To->Show All Connections, right-click on T-Mobile US and choose Properties. On the General tab there should be a checkmark next to Standard Modem (COM4), a checkmark next to "All devices call the same numbers", and the phone number should be *99#
