Sprint offers AIRAVE, Verizon offers Network Extended, basically both are router like devices that create an in-home tower. ATT's service is rumored to be forthcoming. Not to mention plenty of foreign operators offering UMA, so far T-Mobile's in the best and cheapest and doesn't require proprietary hardware that can be costly.

In-home is okay, but it's not enough for me. T-Mobile's UMA works on a WiFi connection anywhere. I get UMA over my home WiFi router, but also WiFi routers at Starbuck's, Borders, Corner Bakery, my buddy's house, my local church, and via a pocket router that I take with me whenever I travel.

Exactly, that is why I love TMO's UMA feature. Work, home, the cafe, wherever I can keep a low minute plan and just have unlimited data to use UMA calling. In the long run the unliminted data rate is cheaper for me than an increase in minutes.
