My company actually pays for tethering, so most of my experience is with the ACM program AT&T makes you go through.
That said, I did dabble with DM tethering before I have it actually put on my plan. I really didn't see any differences, except it was a bit more hokey to get everything working right.
The providers won't turn a blind eye if you start using a lot of bandwidth. Not sure what "a lot" is though.
Most if not everyone who buys tether, do so to bypass having to pay for tethering. I have seen from time to time people get caught (nothing lately however) and have to pay a huge bill.
I'm not a fan of the tether app for reasons I'm not going to go into here and start a flame discussion. However, if it works for you, continue to use it as it is harder for the providers to sense that traffic compared to tethering using DM.
