Again, I cannot stress how important it is to make sure you have all lowercase letters! Be sure to disable Suretype because that caused me to have the problem you are having for 6 hours. Your username and password should be as follows:

MIP yourmdn@bb.alltel.net
Also, double check your BB registration ID. It should be set to 144 not 2004 which is Verizon's. If 2004 is still in the list then delete it.
