You may know most of this but essentially you have 3 main ways to install an app:
(1) Desktop/alx file. You download the app's alx and related cod file to your computer and install the app through Desktop Manager (DM==>Main Menu==>Application Loader==>Add/Remove Applications==>Start). Browse for the alx file....etc...etc..
(2) OTA, or Over the Air. This method you just click a link in your browser and a jad file is loaded....installation follows. Here's a good reference point for OTAs:

(3) Card install. In this case, you need to place a jad file for the app on your card. To do this, download the app's alx and related cod file(s) as well as a program named Genjad4bb (can be found through a Google search). Open Genjad4BB, navigate to where you have the alx/cod files for the app (i.e., files for a desktop installation), and generate a jad file from there. Move the folder with the jad (also contains one or more cod files) to your card and put it back in your BB. Then, using a free app like BBFileScout (listed in the link above), navigate to where you have the folder with the jad and cod files. Click the jad and the installation should be triggered. [Note: Genjad is hit or miss for me....not all jad files actually work.] Why do this when an app can be installed OTA from places like the link above or the CB App Store? Some apps only offer desktop installation, with no OTA option (or the OTA link is down). The ability to install an app without connecting to DM to install the alx is helpful.
My main approach for installing apps was to create a Word document with OTA links for my apps (free and paid). After an OS change, I can just open the doc and click the OTA links to install....on by one (that is, if I don't do single restore of apps through DM). This doc is, in effect, a one-stop shop for me similar to the CB OTA link above under #2.
