What does it really mean?

That app gets access to my picture videos including cloud storage?

Or it is just get access to write to Sd card for its cache and other images that the app will download?

normally a shared files give yyou acces to the things you have on your device , normally not the cloud unless you have your dropbox account there and what to shared something from there

It means that the app can access the content you see when you connect your device via USB. It doesn't grant access to everything on the device.

Apps usually just want to cache stuff here.
