I'm trying to find an app for the Playbook in which i can complete a model release form while on location. I'm a video producer and when ever we film a member of the public we need them to sign a form giving us permission to use their footage in the production. Up until now we use a paper form, which is fine until you have 30 or 40 of them to keep hold of and file afterwards. What I'm after is some way of doing this on the Playbook.

I need to....
- Get the model to read a paragraph of text
- Take their name, phone number and email address.
- Attach a photo of them that is taken by the Playbook's camera
- Sign the form using either a stylus or their finger on the touch screen to say they understand the text.

I know there is an app for the ipad called Easy Release but so far I've found nothing for the Playbook.

Does anyone know of anything out their or an app that i can easily design something that can do all of the above?

There's an app called Canvas in app world.

It lets you make the type of form you need...a waiver with image capture

But I think it's a cloud-based service. Maybe you can make an app for the playbook with a one-time fee
