There are several threads on this issue. Search the forums and you'll find lots of guidance.
It seems that other users have found that reloading their OS doesn't help -- that it's a hardware issue. Apparently you have a good camera or you don't. If you got the phone new, contact your carrier for a replacement.

I bought the phone new the box was still sealed but had it shipped over from the states to the UK, it seem's to be fine with OS5 and have tried several different OS6 and is the same
If you think that the camera works with OS5, then perhaps that's your answer. Most folks with the horizontal lines problem have found that an OS reload is maybe a short-term fix, increasing the liklihood that the problem is hardware. That's not to say your issue can't be different. If you use the camera often, you might be happier with OS5.
