We need a little more information on this one...
What case or holster are you using? Does it have a "sleeper magnet" in the case? What carrier and OS do you have? Does the phone "come to life" when taken out of the case?

I've read some reviews of certain casses taht press on both the lock and mute buttons at the same time and this causes the phone to lock up. What case are you using? More detail is needed to really be any help.

Had the same problem with my case-mate carbon-fiber case... I sanded the top part down, and everything is working fine now!

I beleive the reason behind this is because it holds the "mute" button down and locks up the phone, only a battery pull would fix the issue... then I realized that the case was a bit too tight on, so sanded and everything is fine now!

Yes, sanded the part that was making contact with the button, and voila, no more problem since!

With the Otterbox Case you have to careful not to get the silicone liner bunched up, at the top, when you are pushing the phone into it or it can push up against the top phone buttons.
    It's a good case though.

With regard to people saying the Otterbox case from AT&T store (Commuter) causes problems, I have never experienced this. Yhe only issue I have, which is a small one at that (for me), is that the piece that covers the screen bezel doesn't line up perfectly with the rest of the case when not slid out (i.e. keyboard exposed). Makes pushing the call/menu/back/end keys a little strange and undesirable, but never fully nonfunctional.

I've had the OB since September, and never had a device problem from the cover. I've actually had 3 replacement bezel covers - the verrrrry thin plastic clips that retain the bezel cover break easily. Once the clips break the bezel cover won't stay on. But OB has protected my Torch from numerous hard falls, so I think it's done its job. My local AT&T store has replaced it each time, no questions asked.
