When getting a new phone number, does any carrier let you pick a custom number? I don't mean StarStar vanity number like **Chris. I mean something like (201) 765 - 9999, or something like that where you get to pick the last 4 digits?

I don't believe carriers let you pick the last four but you can pick the area code and the 1st 3 numbers

Back in 1998,before phone # portability, I started a new contract with Sprint . I asked the rep at the store if I could get my current # switched/transfered to my new Sprint phone. He said no ,because the prefixes were assigned to certain carriers.but he did let me pick one from a list of availiable #'s from there network in the area.

AT&T has let me pick my last 4 on both lines I've set up with them.

^^^^This. when I first got a cell phone, VZW let me choose the last 4 digits. I asked for something "easy" or repetitive and they let me choose. I don't know if that is available anymore.
