    IT HAS GOT NOTHING TO DO WITH THE SD CARD.

    You will get this "Unknown" folder if you have certain side loaded Android app that uses/store images.

    Android apps store images in the built in memory.

    You will have to reload the OS or do a security wipe to solve the issue.

    Yes you're right. I recently upgrade my sd card and the unknown album is still there, hopefully an os update will also fix this.

    The Unknown Folder is created in the SD Card itself. Copy the full content of the SD card in a computer, format the SD Card(from Mobile Settings Option) and copy back the content to SD card again. And Voila!!

    You will not be able to permanently delete it (phantom or excess folder) if any app (Android and not only sideloaded but official android) needs that folder or the files. It will probably return. It is the consequence of the way the phone (or playbook) reads/indexes the files needed. Certain apps should be designed to screen these folders/files out but they don't. Whether it will be fixed - anybody's guess.
