Have you rebooted? Have had it about two times during the last 6 months. Rebooting solved it for me.

So here is a question. If it is just sitting out untouched does it ring every time? The reason I ask is if the screen is touched at all when it is ringing it will silence itself. Is it possible that you phone is in your pocket getting touched without you noticing and making it stop ringing.
