What is this? It is an app that came from nowhere and it is looking to access information on my phone. It does not let me minimize the app. There is just an ok button. Any ideas?

Just agree with the permissions, the app doesn't do any harm and might even come in handy

It was working fine until I reloaded my OS last night. Reinstalled the app from BBW and now it says not charging when i clearly am charging. In fact, when I click on device manager it takes me to device manager that says CHARGING.

You can test if something is wrong with your device by using this app.
It was already there in your device but it was hidden
