I Have used "freemyblackberrydotcom" many times and have been very reliable.
I have been referring to them since they started with Testing and service was free etc.

I would recommend UnlockPhoneNetwork.com. I just used them for my bb and they are awesome. They respond in a timely fashion and give great support. If you want to research them they are on this site as well. Good luck.

I paid 2 companies and the codes did not work , then the nice people at unlockphonenetwork.com helped me out with no hassle very highly reccommended

I just used "www.freethisberry.com" and it worked perfectly, I got my code with instructions in a few minutes and it was only 8.50 CAD with the coupon. Thanks Paul!
