I dont remember how my FRU box looked like but it should be similar to the sticker when you first bought the phone. The only difference is the top half of the sticker where it says FRU9530 but the part with your phone information such as SIM #, PIN #, MEID and anything below should be the same. I had a photocopy of mine but threw it away.

it has the day the month and then the year. it will either end in XX1108 for November 2008 XX1208 for December 2008 or XX0109 for January 2009

Should be in the bottom right above where it says Handheld Made In...
