    I could not find the actual icon for address book. It showed up in "frequently used" and "favorites' but I can't get it to show up in "ALL" I have "Show all icons" checked and it still doesn't show up!

    Did you put it in a folder? try looking through all of them. If it still isn't there try either a batt pull or a wipe and restore.
