Any ideas?
1. lowercase, as displayed should be fine
2. perfect, that's what it's looking for
3. just the numerical part is fine

How are you handling the html file? Are you opening it in the phone's browser or on your computer? If you're just double-clicking it on the phone and it's opening but not doing anything, there's a chance it's not opening in the browser. The test is that the code at the bottom should update with every key you type.

I just wanted to say that while I have been exploring all the menus in the Engineering Screens I have not found anything that could brick your phone. There are a lot of options and you probably could make some major changes to your phone but I highly doubt someone could brick theirs. Anyways, I have bricked mine twice and I was able to recover it. The only ways I bricked mine was installing an OS I built (I fixes the OS and got it to work eventually) and I also bricked mine when I debranded it (I had many problems in that process).

I found a app for BB called EsCodeGen you can generate your escreen code with it.
No need to go to internet page for escreen code it works great.
Engineering-Screen-Unlock-Code-Generator
I use a lot 3G and inhome my bb switch a lot between EDGE en 3G now I choose UTRAN and it stay at 3G very nice for tethering speed. Also nice when traveling and listen to TuneIn.
I browse all the options and there is NO WAY to brick your phone if you do something with a wrong result do a pull off and all your settings are at DEFAULT so play and find nice things in here.

Enjoy DelftEo
