Sample report from BES5?

Dear All,
We are migrating from Bes 4x to 5.0 and as part of the move I've been asked to engineer a way of exporting assets and info into our servicedesk system.

The receiving system is happy with csv's but we need to do field mapping.

Can anybody let me have a sample output from bes5 please so I can prep the mapping before implementation? - I'm not after your data just field headings :-)

We would be looking at username ,blackberry model, firmware and pin as starters.

I'm working through the Bes guidance but going slightly round in circles

This looks like a link to the BES reporting tool BlackBerry Application Reporting Tool - BlackBerry Analysis, Monitoring, and Troubleshooting Tools - Administration Guide - BlackBerry Enterprise Server Resource Kit - 5.0.1 but I'm not sure where the tool the page reference actually is.
