Never got a response but for anyone that runs into this issue, I think I found the problem. My account in AD was listed in the Bulliten group and Administrators group. These are protected groups that sometimes reset. So the BESAdmin account that I kept configuring with a send as permission kept erasing itself.
I had to go in and stop the blackberry router, add the BESAdmin account again with the send as permission in the security properties tab of my account in Active Directory. I removed myself from the Bulletin Administrators group. I dont think you have to do that, I think there is a script or something you can run but I didnt need MY account to be a domain admin. I just log in as an actual Administrator instead of my account. After setting the BESAdmin account with the send as, I restarted the blackberry router service and after about 30 minutes it clears and I am able to send and receive.
