There is an entry in my call log #PA* but the P is white in a square - see attached picture. I tried to recreate this but I cannot find the key or symbol that can create this type of P entry. I did not type this in and I am trying to figure out if I leaned on the keyboard and created this entry or somehow my phone picked it up.

That's the icon for a pause in the dialing stream. Press Menu while entering a number and you get an option to insert pauses or waits.
