Default Newbie with complex problem

Sorry if this has been posted already but I couldn't find anything about it. My 8830 from sprint seems to be stuck on Extended Network in a very good coverage area. I have other sprint phones right next to it that are not roaming and have at least 4 bars. Because of this issue I can not get the provisioning to complete to get my data service working. I've spent hours on the phone and at the sprint store, done all the usual steps. It has the OS version 4.5.0.186. I can send and receive calls and texts with no problems. Is this perhaps a common issue or more likely a device defect? Broken antenna maybe? Provisioning fails with the 1012 error code.

Have you tried setting the network to Home Only?

Setting it to Home only makes it search for CDMA continually. Even in a good sprint location it never finds the sprint network. I've updated the phone OS, wiped it several times, pulled the battery I don't know how many times.
