    It can only be activated via your carrier/network

    Same carrier and same number, I believe yes. Can not be RE used to port a new number to it or used in a new network.

    It's dependent on the carrier you're with. Generally speaking, you can reactivate a SIM card so long as it is being activated on the same account, with the same line it was originally activated with. Some carriers will not let you reactivate an old SIM card if it has been unused for a certain period of time (this can range from 6 months to a year).

    I would advise contacting your carrier to see what they are able to do for you.
