If you reinstalled the OS and didn't restore any data... I'd say you have a hardware issue. You could try installing a different version of the OS to rule that out....
The Z30 camera module is replaceable... if it's worth it to you. MIght could disassemble the phone and see if the connection has just come loose.... Do some searches on Youtube to see how to do it.
